HEALTH CAREERS PROGRAM - Program Forms

We offer programs for students through the academic school year as well as through the summer months.  Below you will find applications to complete and submit.  Please note, that these are online applications, and must be completed in one browser session.  Progress cannot be saved and if the browser closes, you will have to start over.

2021 Pee Dee AHEC High School Summer Health Careers Exploration Application

This program is a one-week experience, wherein high school students (rising 9th graders through graduating seniors) will have the opportunity to explore the different career options in the field of healthcare. Students will spend a portion of each day in different clinical and non-clinical units where they will observe the policies, procedures, illnesses, and preventative measures taken, while simultaneously gaining exposure to the career details associated with healthcare professions. Student participants will receive advice concerning career and college readiness from real-world healthcare professionals, as well as a better understanding of the variety of healthcare professions through hands-on career labs. This program will operate during the second and third weeks of June.
The total number of hours earned for this program will be 45. A certificate verifying participation will be presented at the end of the program.

2021 Pee Dee AHEC Undergraduate Summer Internship Experience Application

The Pee Dee AHEC Undergraduate Regional Internship Experience is a 4-week program that offers students the opportunity to intern in one area or explore all available areas of healthcare. In both the clinical and non-clinical units interns will observe policies, procedures, preventative care, and treatments. Students will have the opportunity to collaborate with peers, explore healthcare careers, and discuss their experiences and insights.

The internship will begin July 5th, 2021 and end July 30th, 2021
Application deadline is April 15th, 2021

Eligibility:

  1. 18 years or older enrolled in an underfraduate program with an interest in healthcare.
  2. Applicants must have the understanding that internship sites will be located within Pee Dee AHEC's region.
  3. Complete and present a professional culminating presentation.
  4. Complete weekly journal experience assignments.
  5. Application deadline is April 15th, 2021.

Intern Requirements:

  1. To complete 80 hours over a four-week period.
  2. Attend meetings outside of the designated 80 hours.
  3. Complete and present a professional culminating presentation.
  4. Complete weekly journal experience assignments.
  5. Application deadline is April 15th, 2021.

The application process for the Summer Internship program is competitive. Submission of the application does not guarantee acceptance into the program.
